Coordinated Healthcare Interventions for Childhood Asthma Gaps in Outcomes (CHICAGO Trial)

Chicago is an epicenter for asthma health disparities in the U.S., with African-American
children 5-11 yrs bearing a disproportionate share of the burden. Among the most visible of
these disparities is the high rate of visits to the Emergency Department (ED) for
uncontrolled asthma. Clinical uncertainties regarding the real-world effectiveness of
guideline recommendations for ED discharge and strategies to reduce environmental triggers
at home contribute to practice variation and poor adherence to guidelines. The CHICAGO Trial
tests both ED- and home-level interventions to improve clinically meaningful outcomes in a
minority pediatric ED population with uncontrolled asthma.

Inclusion Criteria:

- Age: 5-11 years at date of ED visit

- Seen at participating ED

- Attending physician clinical diagnosis of asthma exacerbation

- Treatment with at least 1 dose short acting bronchodilator (beta2 agonist or
anticholinergic)

- Treatment with at least 1 dose systemic corticosteroids (oral, IV or IM)

- Receiving treatment for asthma in ED and/or observation unit for at least 1 hour

- Attending physician indicates that patient is likely (>75% chance) to be discharged
to home directly from ED or observation unit.*

- Primary language: English or Spanish

Exclusion Criteria:

- Patient admitted to the hospital

- Discharge location other than home (e.g., other hospital)

- Caregiver unable or declines to provide written permission or informed consent prior
to discharge to home

- Child or other member of household is current/previous participant in the CHICAGO
Trial

- Concurrent enrollment in other asthma research or enrolled in a different Community
Health Worker (CHW) home visit program.

- Plan to move away from study site in next 12 months.
